Will you have access to a technological device (e.g., computer, smart phone) that can access the online intervention via a web browser throughout this 14-week study? Please know that for some people using a device with a large screen (e.g., computer) may be more comfortable throughout the study.*

Do you agree to not enroll in another intervention program (not counting the weight management program provided by the center) promoting physical activity while enrolled in the current study?*

You will be asked to wear a nylon belt around your waist with a physical activity monitor attached to it for 2 weeks (e.g., 1 week at the beginning of the study + 1 week at the end of the study). You will be asked to visit the Lourdes weight management center to receive or return the physical activity monitor. Do you agree to comply with instructions for physical activity monitoring in this study?*
